GBHD Enrollment Forms

To view and obtain relevant forms please click on the corresponding links in the menu bar on the left side of this page. 

Enrollment Packet: All required information and a form a the end for your child's primary care physician to complete.  I also need a copy of your child's most recent physical exam, lead screening and vaccination records.

Developmental History Form: It is important for me to know if there were any complications or abnomalities with your child's early development.

Toothbrushing Form: Grant permission or waive opportunity to have your child brush his/her teeth after lunch. 

Photo Permission: Allow for you child's photo to be taken and used for advertising and display in the daycare.

Consent to Treatment: Allow for your child to be given various first aid treatment such as tick removal and eye flush.

Sunscreen Form: Permission to apply sunscreen and insect repellent.

Medication Authorization Form: GBHD requires permission to apply anything on or administer to your child.  The form is for non-prescription and prescription medications, ointments and creams.  Please review the form for additional information and consent requirements.  It is recommended that you keep a copy of this form in your diaper bag or car, in case your child's doctor prescribes medication at a visit that will need to be given or applied at the daycare.  A completed form can also be faxed to the daycare if needed.

Pick Up Authorize Form:  Please obtain and complete this form if someone other than yourself or a pre-approved person is going to pick up your child.  The form must be given to GBHD prior to your child being picked up.  GBHD will require that the person show a photo ID before releasing your child to them. 

Request for Notification Form:  Do you want to be notified any time I apply something on your child (i.e. diaper cream).

Enrollment Contract:  This is GreenBaby Home Daycare's contract for care.  The contract runs from September to June.  A new contract will be required each September that your child is enrolled at the daycare.  If you enroll your child after September, the contract will be in effect from the first day of care until June.  Please review the agreement for other important information regarding terms of care.

Intent To Withdraw:  Please obtain and complete this form if you wish to withdraw your child from GBHD.  A 30 day notice is required to apply your $100 tuition fee to your child's last month of care.

Daily Note: used to record all the daily activities, food served for snacks and lunch and nap times. 

Tuition Invoice: used weekly to calculate your child's tuition.
